At Dr. Traci’s House, Patient Rights and Responsibilities are an important part of providing community-based health care. Here are some important principles you should know about.

STATEMENT OF PATIENT RIGHTS

You have the right to:

1. Considerate and respectful care.

2. Obtain from your physician complete and current information concerning your diagnosis, treatment, and prognosis in terms, which are easily understood. When it is not medically advisable for you to receive such information, the information will be made available to an appropriate person in your behalf. That individual has the right to know by name, the physician responsible for coordinating your care.

3. Receive information about any proposed treatment or procedure as needed to give informed consent or to refuse this course of treatment. Except in emergencies, this information will include a description of the procedure or treatment, alternate courses of treatment or non-treatment and the risks involved in each. You also have the right to know the name of the physician or medical staff member who will carry out the procedure or treatment.

4. Every consideration of privacy concerning your medical care program. Case discussion, consultation, examination, and treatment are confidential and should be conducted discreetly. Staff not directly involved in your care must have permission to be present.

5. Obtain information as to any relationship of the clinic to other health care and educational institutions involved in your care. You have the right to obtain information as to the existence of any professional relationship among individuals, by name, which are part of your treatment team.

6. Expect reasonable continuity of care and the right to know in advance what appointment times are available and the location of available physicians.

7. Reasonable requests for services. The clinic must provide evaluation, service, and/or referral as indicated by the urgency of the case.

8. Expect that all communications and records concerning your care and visit to the clinic will be treated confidentially. Written permission shall be obtained before your medical records can be made available to anyone not directly concerned with your care.

9. Have all the patient rights apply to the person who may have legal responsibility to make decisions regarding medical care in your behalf.

10. Leave the clinic even against the medical advice of your physician or clinic staff.

11. Know what clinic rules and policies apply to your conduct as a patient.

12. Express dissatisfaction, without concern for jeopardizing the quality of your care, by communicating concerns or complaints to the Clinic Director of Operations or designee, or the Medical Director. Each department posts the following statement in its public waiting area if such an area exists: Dr. Traci’s House values patient feedback related to the services we provide. We believe that this important information represents opportunities to improve the care we offer. Accordingly, patients and families are encouraged to express concerns, complaints and compliments to the supervisor of the department in which patient is receiving care, or Compliance Officer at (813) 501-7796 or to provide written feedback to the above individuals at: Dr. Traci’s House, 904 E. 13. Examine and receive an explanation of your bill regardless of source of payment.

14. Exercise these rights without regard to sex or cultural, economic, educational, or religious background or source of payment of your care.

STATEMENT OF PATIENT RESPONSIBILITIES

1. Providing the necessary personal and medical history required for treatment.

2. Advising the staff whether you clearly understand a medical procedure and what is expected of you.

3. Following the prescribed treatment plan recommended by your physician, which may include following the instructions of nurses and other health professionals involved in your care.

4. Keeping appointments, and when unable to do so for any reason, for notifying your physician or the clinic.

5. Your actions if you refuse treatment or do not follow your physician’s instructions.

6. Assuring that the financial obligations of your health care are taken care of as promptly as possible.

7. Following clinic rules and regulations affecting patient care and conduct.

8. Being considerate of the rights of other patients, clinic personnel, and for assisting in the control of noise and number of visitors. In addition, you are responsible for being respectful of the property of other patients and of the clinic.
